Hi, I am Chioma and I am eager to present a detailed evidence-based population health improvement plan particularly designed to address mental health illness such as depression across black populations in United States. This presentation will investigate community data, environmental challenges affecting community and interventions to address disparities and challenges associated with depression in this population.

Despite long term efforts to combat depression and associated challenges in United States, health disparities persist in the community due to shortage of mental healthcare workforce, ethnic or racial discrimination, differences in socioeconomic status and geographic location at community level (Whitman et al., 2022). Government institutions and healthcare organizations need to adopt evidence-based population health improvement plan to address mental health issues and related disparities. This strategy uses data from previous five years researches to identify potential community needs and barriers in the implementation of plan to meet those needs. The aim is to evaluate mental health disparities and emphasize on evidence-based strategies to improve treatment plans and ethical considerations. This strategy assists in measuring successful outcomes and implementation through communication engagement of stakeholders in the community.

  • Community Data Evaluation

Evidence -based discrimination attached to black Americans interfere with help seeking, treatment options and recovery, workforce development and overall life productivity of individuals with mental illnesses (Pescosolido et al., 2021). Several social, economic and environmental factors contribute to increased prevalence of mental illnesses such as depression. To reduce rates of higher prevalence, it is necessary to address the complexity of these contributing factors.

In recent statistics from Office of Minority Health, (2024) reported that African American received mental health treatment 36% less than overall U.S. in the past year. Moreover, African Americans are 8% more likely to die from attempting suicides than others (Office of Minority Health, 2024). The increased prevalence of major depressive disorders among black American populations in United States points the constant management system. These findings suggest the evidence-based health improvement plans to address high rates of depression and improve overall quality of healthcare services among African Americans.

  • Environmental Factors affecting Community

Environmental factors play a significant role in increasing the prevalence of mental illnesses across black populations of United States. Common environmental factors such as chronic stress, poor dietary habits, low socio-economic status greatly impact the incidence of mental illnesses across specific population (Kirkbride et al., 2024). Excessive workloads and domestic violence can worsen the high rates of depression and related issues.

  • Ethical Health Improvement Plan

To reduce high rates of mental illnesses among United States, particularly in black Americans, evidence-based mental health plan is imperative. To address challenges ethically and inclusively, multifaceted approach must be employed. It includes environmental interventions to meet physical community needs and customized education in addition to support services. Culturally and linguistically boost engagement of patient and family members is also crucial in health improvement plan. According to structure of comprehensive mental health action plan 2013-2030 by World Health Organization (WHO) mental health disorders are prevented to value mental health. Persons affected by mental health disorders are able to experience full human rights, high quality health care, culturally appropriate and social care on time to promote recovery (World Health Organization, 2023). Moreover, its aim is to attain highest possible reduction in social, environmental and cultural disparities without any stigma and discrimination. The plan aims to enhance healthy outcomes with reduction in higher rates of mental illnesses to empower black populations of Americans in United States.

  • Barriers Regarding Implementation

For implementation of mental health improvement strategy, it is necessary to identify and address potential mental health needs and social determinants of mental health. Building competencies is imperative to better understand needs of people who deserve mental health improvement plan and its implementation (World Health Organization, 2022). The needs acts as potential barriers if not address in vulnerable people. A report from World Health Organization. (2023), challenges in implementation of health improvement plan may include poverty, limited access to resources, ethnic or racial discrimination, domestic violence and inadequate behavior of healthcare providers when responding to person with mental health illness.

  • Evaluation of Outcomes

Comprehensive evaluation of health improvement interventions impact black Americans in achieving excellent health outcomes. Key indicators in evaluation of successful measures include improvements in patient adherence to treatment, self-described health outcomes, and reductions in high rates of depression (Dizet et al., 2025). Additionally, reduced emergency visits to healthcare centers and hospitalizations related to severe depressive disorders will indicate progress in better management and its impact on healthcare utilization. To identify effective strategies, allocation of resources more effectively and assistance of stakeholders to access progress, regular monitoring of evaluation criteria is required. It is also necessary to address challenges and disparities in healthcare within the community.

  • Community Collaboration Plan

To address mental health illness effectively, collaboration with organizations such as Veteran of Foreign Wars (VFW) will be targeted approach for reducing rates of depression and increasing community engagements successfully.

  • Strategies for a Respectful Exchange of Ideas

VFW will be conducting joint community needs assessments in partnership with healthcare organizations to guarantee valuable insights into development of our plan. They also deliver cultural competence guidance to healthcare clinicians to substitute mutual understanding and respect (Mental Wellness, n.d.). The aim of this approach is to build a comprehensive collaboration that compliments unique cultural differences and perspectives within community, improving the efficiency of mental disorders management across healthcare system.

  • Relevant Community Stakeholders

In population health improvement plan active involvement of VFW, local clinics, hospitals, and faith-based organizations act as key stakeholders. The plan will emphasize on integrating joint needs assessments with the VFW to ensure ethical and culturally sensitive collaboration, (Mental Wellness, n.d.). The aim of this approach is to establish a collaboration that values diverse perspectives of black Americans, thereby enhancing the effectiveness of mental healthcare interventions in the community.

  • Communication Strategies for Data Privacy

There is need of platforms for secure communication among patient and healthcare professionals which protect data of patients. Confidentiality and security of data are among few measures to ensure data protection (Conduah et al., 2025). Here a question arises on how and why data will be shared among organizations. To answer such questions on data protection, one of the essentials of the plan are consent forms for regulating data protection. For transparency in healthcare system, patient compliance and reducing the legitimate risks and financial burdens, consent forms safeguard data privacy as well as reliability of organization (Shah et al., 2024). These communication measures can empower ethical standards, privacy respects and trust worthy relations among patient and healthcare professionals within community. To communicate effectively about healthcare issues within community, it is imperative to translate educational material in native languages for better understanding of complex medical terminologies. Digital technology tools such as multimedia content and graphics will simplify the complex concepts for learning preferences. These strategical interventions will minimize the gap between individuals from diverse language backgrounds and healthcare professionals improving health outcomes of community.

  • Value and Relevance of Evidence and Technology Resources

The evidence-based population health improvement plan is necessary for addressing mental illnesses such as depression. According to WHO findings of 2023, 1.1 billion people are affected by depression worldwide. This highlights the importance of addressing mental health issues globally. Integration of guidelines provided by WHO in health improvement strategies will result in better outcomes within community (World Health Organization, 2023).

Health literacy and positive outcomes can be significantly achieved through digital technology tools such as mobile health apps, AI powered software for better for better evaluation and management of mental health issues. According to study findings by Forman-Hoffman et al., (2021), significant reduction in number of emergency hospital visits results from use of depressions related apps because these technology tools assist in customized access to strategic interventions allowing real-time data exchange while maintaining privacy of individuals and integrity of healthcare organizations.

By utilizing evidence-based practices, developing collaborative partnerships with organizations like VFW, and addressing ethical sensitivities, the plan aims to improve health outcomes within the community. The plan seeks to address the complex nature of depression through analyzing community data and identifying implementation barriers. These improvements assist in reducing high prevalence of mental illness such as depression among African Americans.
